Original Medicare

Original Medicare is the traditional federal Medicare program: Part A for hospital coverage and Part B for medical coverage. Understanding what it pays — and what it leaves to you — makes the rest much easier.

What Original Medicare includes

Part A generally helps cover inpatient hospital care, skilled nursing facility care under qualifying conditions, hospice and some home health care. Part B generally helps cover physician services, outpatient care, preventive services, durable medical equipment and other medically necessary services.

Why people like Original Medicare

Broad access to providers that accept Medicare
No Medicare Advantage network to stay within
Usually no referral requirement to see specialists
Works nationwide with participating Medicare providers

What it does not solve by itself

Original Medicare has deductibles and cost sharing. For many Part B services, after the deductible, you generally pay 20% of the Medicare-approved amount. Most importantly, Original Medicare by itself has no annual out-of-pocket maximum for Part A and Part B services.

A common misunderstandingOriginal Medicare does not normally include routine dental, routine vision, hearing aids or most outpatient prescription drugs. Those gaps are one reason people consider Part D, Medigap or other coverage.

How Part D and Medigap fit

CoverageWhat it does
Part DAdds outpatient prescription drug coverage through a private Medicare-approved plan.
MedigapOptional private supplemental insurance that helps pay some of your share of Original Medicare costs.
Mike & Tracy's tipWhen comparing Original Medicare + Medigap + Part D with Medicare Advantage, compare the entire package. A higher monthly premium can sometimes buy more predictable medical costs; a lower premium can mean more pay-as-you-use-it cost sharing.

Who might appreciate this structure?

People who highly value nationwide provider flexibility, travel frequently, see specialists in multiple health systems, or prefer fewer network restrictions often want to understand Original Medicare carefully. That doesn't automatically make it the best choice — budget and supplemental coverage matter.
